To get started simplifying your business procedures, saving your practice time and money and succeeding at
growing your practice and your bottom line, please complete these simple steps:
- Download the membership application.
- Complete the membership application on your computer.
- Print out the completed application and endorse with your signature.
- Fax the completed and signed application to 1.507.629.3388

Once your faxed application has been received and we gain credit approval, your practice will be notified
