Add a Vendor to Your ADO Account
To add a new vendor to your existing ADO account, please complete these simple steps:
- Download the Vendor Registration form.
- Print out the Vendor Registration form.
- Complete the Vendor Registration form.
- Fax the completed and signed form to 1.507.629.3388

Once your fax has been received and you vendor requests added, your practice will be notified and given your new account numbers. If you
